Registration:

The Washington Local School system provides elementary and secondary education (K-12) for children meeting residency requirements. In order to be eligible for public education, a student must be in legally recognized custody of a resident or government agency within the district.

Registration for all students grade Kindergarten through 12 is done at the Washington Local Board Office, located at 3505 W. Lincolnshire, Toledo OH 43606.
Registration is by appointment only, Monday through Friday 8:15– 2:30.
Please call 419-473-8296 to schedule an appointment.
You must bring all required documentation to your appointment or your appointment will be re-scheduled.
The required documents are:
_____Completed Enrollment Packet
_____Official Birth Certificate (No photocopies accepted)
_____Parent/Guardian Photo ID
_____Custody Papers (if applicable)
_____Proofs of Residency (2 Required)
_____Friends and Family Affidavit (if applicable)
_____Special Ed IEP and ETR ( if applicable)
_____Previous School Transcripts or Grades, with completed request

See details below:

1. Enrollment Packet – Please complete the forms supplied in the packet. If you have questions the enrollment office will assist you.

2.Official Birth Certificate with raised seal - If you do not have an original you may obtain a certified copy of a birth certificate through the Toledo Lucas County Health Department – Vital Records Department at 635 N. Erie St, Toledo Ohio 43624. There is a $25 fee. Please call 419-213-4100 for details.
3. Photo ID of parent/guardian

4. Custody Papers - If custody is not with biological parents you must present legal custody papers.
If parents are divorced you must provide journalized, signed custody papers proving residential parent information.
If legal custody has been transferred you must provide a stamped court document.
If legal custody has not been established you must present a court stamped motion for change of custody.

5.Proof of Residency – Washington Local does not have an open enrollment policy therefore you must be a resident of the district in order to attend. As proof of residency you must provide two of the following:
Current utility bill (gas, electric, water, cable) STUB MUST BE ATTACHED
Signed lease agreement (must be current)
Signed purchase agreement for a home
Government agency documents (for example, unemployment statements, job and family services, social security administration)

6. Friends and Family Affidavit – complete if parents/guardians are doubling up.
For persons living with family or friends: completed by both the guardian and resident, and notarized. Resident must provide two proofs of residency and guardian must provide one proof of residency. If the resident's home is a rental, the guardian's name must be listed on the lease.

7. Special Education - If your child receives special education services you must bring the current signed IEP (Individualized Education Plan), and signed ETR (Evaluation Team Report). Your child can not start school without the IEP and ETR.

8. Previous School Information - Please bring the following to your scheduled appointment:
Current transcript (High school only)
Name, address, phone number of previous school
Most recent grade card

Kindergarten Registration: Washington Local offers all-day kindergarten.

Kindergarten is mandatory in the state of Ohio; any child entering first grade must have successfully completed kindergarten. Children who will be five years old on or before September 30 should enroll in kindergarten. The child must be registered by a parent or legal guardian, who must present an original birth certificate, 2 documents showing proof of residency, court-stamped custody papers (if applicable), and immunization records.

Immunization Requirements:

According to the Ohio Revised Code:

  • 5 diphtheria, pertussis, & tetanus (DPT)
  • 4 poliommyelitis (polio)
  • 3 hepatitis B
  • 2 rubeola (measles), mumps, and rubella (German Measles)(MMR)
  • 2 varicella (chicken pox) vaccine - effective for kindergartners entering Fall, 2010
  • 1 tetanus, diptheria, pertussis (T-dap) - effective for 7th graders entering Fall 2010

Transferring Within the Washington Local / Intradistrict Transfer Procedures:

District residents who would like their children to attend a Washington Local School other than their home school may request transfer consideration by submitting an application no later than April 25, 2013 (exceptions apply). These applications are for regular education students (including students with a Speech Only IEP). All completed applications should be sent to the office of the Assistant Superintendent at 3505 W. Lincolnshire, Toledo, Ohio 43606. Transfer decisions are based upon building and grade level capacities and other established criteria. Preference for enrollment in any particular school is given to students living in the school's established attendance boundaries.

If your child has special needs or is on an IEP (other than Speech Only), you do not fill out an application. Please contact the Student Services Department at 419-473-8236. Transfer decisions for special education purposes continue to be based on state regulations and Individual Educational Plans.

All approvals for intradistrict enrollment will be reviewed annually. Therefore, parents or guardians whose intradistrict transfer requests were approved for the 2012/13 school year must re-apply if they wish to continue sending their child to a Washington Local school other than their school of residence.

If the transfer request is granted, student transportation will be the responsibility of the parent or guardian. Parents or guardians who would like their students to continue attending their home school, need not apply.

Washington Local's intradistrict enrollment policy is in compliance with the state law which requires school districts to establish limited transfer opportunities within the school system. Decisions on intradistrict transfers will be made in mid June. In cases where there is the potential for overcrowding, decisions may be delayed until the opening of school in August 2013.

Special Education Students - Required Documents

In order for our staff to serve the special needs of your student, it is vital that we have the most recent copies of the following documentation:

ETR – Also known as MFE or Evaluation Report. The title of this document varies from state to state. This is the document that shows your student qualifies for special education services and highlights the areas of need. This evaluation is done every 3 years.

IEP* – This is the student’s Individualized Education Plan and contains goals and services that support your student’s specialized needs. IEP’s are done every year.

* If your student is transferring to Washington Local from a parochial school, the student will have an SP (Service Plan) in place of an IEP. Service plans are also done annually.

Having these documents with you at the time of registration will help us place your child in the appropriate educational setting and allow your child to start school promptly, and in the correct program.
