Kindergarten Registration for Fall 2013
Registration for all students grades Kindergarten through 12 is done at the Washington Local Board Office, located at 3505 W. Lincolnshire, Toledo OH 43606.
Kindergarten information night will be held at Meadowvale Elementary School, 2755 Edgebrook Dr. on March 21, 2013 at 7:00 p.m. Registration packets will be available and you may sign up for an appointment for your May registration date. If you are not able to attend Information night, you may call your home school to schedule a registration appointment.

| If your child does not register in May, registration is by appointment only, Monday through Friday 8:15 – 2:30.
Please call the registration office at 419-473-8296 to schedule an appointment.
You must bring all required documentation to your appointment or your appointment will be re-scheduled.

Kindergarten is mandatory in the state of Ohio; any child entering first grade must have successfully completed kindergarten. Children who will be five years old on or before September 30 should enroll in kindergarten. The child must be registered by a parent or legal guardian, who must present an original birth certificate, 2 documents showing proof of residency, court-stamped custody papers (if applicable), and immunization records.

Proof of Residency:
Acceptable documents for proof of residency (2 required):
- a sales agreement for a house
- an official lease or rental contract
- a current utility bill listing parent or guardian's name and address
- government payment (i.e. WIC, Healthy Kids, child support payments

Immunization Requirements:
According to the Ohio Revised Code:
- 5 diphtheria, pertussis, & tetanus (DPT)
- 4 poliommyelitis (polio)
- 3 hepatitis B
- 2 rubeola (measles), mumps, and rubella (German Measles)(MMR)
- 2 varicella (chicken pox) vaccine*
*effective for kindergartners entering Fall, 2010
